New immunization requirements for fall
(May 6, 2013) – Local public health nurses are reminding parents of some changes to the immunization requirements for school entry that will take effect this fall.
Beginning in the 2013-2014 year, the Indiana State Department of Health’s list of immunizations required for school entry will include:
- Two (2) doses of varicella (chickenpox) vaccine, or evidence of immunity, will now be required for all students in grades kindergarten through 12.
- One (1) dose of a meningococcal conjugate vaccine (MCV4) is required for 6th grade.
In addition:
- One (1) booster dose of MCV4 is recommended for all students in grades 11 and 12.
- Two (2) doses of Hepatitis A vaccine (HepA) are recommended for all students entering kindergarten.
While the last two are only recommendations, it is likely that they will become required vaccinations in school year 2014-2015.
The requirements and recommendations are in alignment with the routine vaccination schedules from the Advisory Committee on Immunization Practices (ACIP). The full list of all school immunization requirements can be found online at https://chirp.in.gov/.
Parents needing to get their children these vaccinations should ask their doctor or visit the Fort Wayne-Allen County Department of Health or a Super Shot location.
The Department of Health’s Immunization Clinic is located at 4813 New Haven Ave. The clinic’s hours are 8 a.m. to noon and 1 to 4 p.m. Monday through Friday. Call (260) 449-7514 to make an appointment.
Children under age 18 need to be accompanied by a parent or guardian (with appropriate consent). Children eligible under the Vaccines for Children program may qualify for free vaccine. For all others, there is a $9 administration fee per vaccine in addition to the cost of the vaccine. Cash, debit or credit cards are accepted.

MyVaxIndiana
Hoosiers can access their immunization records through MyVaxIndiana, a user friendly Web site which allows parents and other individuals to look up their immunization records from any computer through the use of a personal identification number (PIN). PINs can be obtained from healthcare providers or the local health department and used to log in to the secure Web site, where users can then download, print or fax the records.
