Lutheran Hospital is seeking input from Allen County residents through a Community Health Needs Assessment survey to gather information on local health issues and how to address concerns.

Public Input Sought for Health Needs Survey

Fort Wayne, Indiana (December 1, 2021) – Lutheran Hospital is seeking input from Allen County residents through a Community Health Needs Assessment survey to gather information on local health issues and how to address concerns.

The brief online survey asks for input on how health issues and needs impact the overall health of the community.

The survey is open through Dec. 21 and can be accessed through the following link: https://www.surveymonkey.com/r/HZ56ZL8 All responses are confidential, and the information collected will be used to help guide Lutheran Hospital in addressing various health needs identified by diverse stakeholders throughout the community.
