Paulding Rd.<br \/>\n    Fort Wayne, IN 46816<\/p>\n<p><strong>WHEN<\/strong>: Monday through Friday 8 a.m. to 8 p.m. <\/p>\n<p>Testing is for anyone living or working in Indiana by appointment only and must be scheduled in advance at https:\/\/lhi.care\/covidtesting  or by phone at 888-634-1116. No symptoms or pre-existing conditions are required. <\/p>\n<p>The current ISDH\/Optum COVID-19 testing location at the New Haven-Adams Township Fire Department, 910 Hartzell Rd, will close Thursday.
