The St. Joseph Community Health Foundation Announces $444,150 in 34 Local Grants<\/strong><\/p>\n

<\/p>\n

(June 27, 2012) – The St. Joseph Community Health Foundation, a sponsored ministry of the Poor Handmaids of Jesus Christ, announces $444,150 in spring grants to local organizations to improve the health, wellness and health care access for the low-income and vulnerable in Allen County from January 2012 to present.<\/p>\n

The Poor Handmaids of Jesus Christ have had active ministry programming in this community since 1868, and through this Foundation have invested $13,264,867 through 854 grants\u00c2\u00a0into 168 Allen County agencies since the sale of St. Joseph Medical Center in 1998. All grants are focused on helping to establish and maintain centers of excellence in health care delivery to the low-income in our community and build programming to address gaps that prevent individuals from attaining health and wellness. In addition to providing grants, the Foundation also provides programming to address gaps in services including operating a Medical Interpretation & Healthcare Navigation Program for Burmese refugees and immigrants, a Resource Directory of Allen County’s low cost, quality healthcare resources; and co-sponsoring the Allen County Lead Poisoning Prevention & Healthy Homes Program.<\/p>\n

The following spring grants were announced:<\/p>\n

 <\/p>\n

Unrestricted Funds
\nAfrican-American Healthcare Alliance of Fort Wayne<\/strong> – $3,000
\nFor equipment, food & nutrition, and educational materials to assess the health of, and teach wellness to, the 600 inner-city athletes involved in the Metro Football League Physicals program.<\/p>\n

AIDS Task Force<\/strong> – $8,000
\nProvide emergency medical assistance to men, women, and children living with HIV.<\/p>\n

Allen County Department of Health<\/strong> – $44,000
\nTo continue providing efficient, timely and accurate interpretation and translation services through two medical interpreters to approximately 290 refugees annually seeking medical and environmental services.<\/p>\n

Cancer Services of Northeast Indiana<\/strong> – $15,000
\nSupport for the Client Advocate program and financial assistance to provide funds to help lowincome cancer patients and their families with prescription costs and medical copays for cancerrelated costs.<\/p>\n

Catholic Charities<\/strong> – $25,000
\nTo provide interpretation, translation, health navigation and case management services for 200 Spanish speaking individuals and families annually to help them access health care & wellness care.<\/p>\n

Clinica Madre de Dios<\/strong> – $10,000
\nOperating support so that quality medical care will be provided to the poorest of the poor in Fort Wayne.<\/p>\n

Community Transportation Network<\/strong> – $30,000
\nTo provide affordable, dependable specialized medical transportation services for seniors, persons with disabilities, and lowincome persons.<\/p>\n

Crime Victim Care of Allen County<\/strong> – $17,500
\nFor the salaries of the Immigrant Addictions Support Coordinators who run support groups serving non-English, non-Spanish speaking refugee addiction clients and their families.<\/p>\n

East Wayne Street Center<\/strong> – $7,500
\nFor the Health Coordinator’s salary, to ensure that Head Start children are up to date on all health screening, diagnostic, and treatment programs, including vision, vaccination, hearing, growth, nutrition, asthma, lead poisoning, developmental and behavioral.<\/p>\n

Family & Children’s Services<\/strong> – $20,000
\nTo provide mental health and substance abuse counseling to approximately 2,300 very low-income
\nfamilies and individuals.<\/p>\n

Fort Wayne Sexual Assault Treatment Center<\/strong> – $5,000
\nTo provide technical support to assist agency in developing a strategic plan for a sustainable aftercare program that integrates behavioral\/spiritual health services toward the whole healing of victims of sexual assault and domestic violence.<\/p>\n

Healthier Moms and Babies<\/strong> – $15,000
\nTo be used as a $1 for $1 matching grant for donations from businesses and individuals to further Healthier Moms and Babies’ mission of improving the outcome of pregnancy and reducing infant mortality among high-risk, low-income families.<\/p>\n

HearCare Connection<\/strong> – $10,000
\nOperating support to help pay for the follow-up care, adjustments and any equipment adaptations for 20 children and 4 adults fitted with free hearing aids in January by a joint effort with the Starkey Foundation Super Bowl Domestic Mission.<\/p>\n

Hope Alive<\/strong> – $15,000
\nA matching grant of up to $15,000 benefitting homeless women, which includes counseling, case management, transitional housing and assistance with finding a job or finishing their education.<\/p>\n

Lutheran Social Services of Indiana<\/strong> – $25,000
\nOngoing support of 550 hours of intergenerational activities through 160 different monthly activities to enhance wellness for 100 children at Children’s Village and 75 residents of Lutheran Life Village.<\/p>\n

Martin Luther King Montessori School<\/strong> – $6,800
\nFor the Health Facilitator, who works to ensure 100 students from atrisk, lowincome families enrolled at MLK Montessori School have basic medical care, are screened yearly, and learn how to be healthy from a very early age.<\/p>\n

McMillen Center for Health Education<\/strong> – $25,000
\nTo underwrite at least 60 programs in schools\/groups where greater than 50% are low-income youth enabling more than 1,600 students, preschool through grade four, improve their health knowledge.<\/p>\n

Mental Health America in Allen County<\/strong> – $20,000
\nTo provide operating support for the client advocacy program to help people with mental health issues access prevention, treatment and recovery programs.<\/p>\n

Neighborhood Health Clinics<\/strong> – $60,000
\nSubsidize dental operations to cover the difference between the copay and actual cost at the clinic.<\/p>\n

SoulMedic Media Group RemedyLIVE<\/strong> – $15,000
\nGive Biblical guidance to the 15,000+ young adults in the Fort Wayne area who go to the website or text with hurts, damaging habits and hang-ups.<\/p>\n

Turnstone Center for Children and Adults with Disabilities<\/strong> – $10,000
\nOperating support to help subsidize 15,300 hours of therapy and 4,500 hours of case management services for children and adults with disabilities.<\/p>\n

Wellspring Interfaith Social Services<\/strong> – $5,000
\nHelp underwrite staffing for the Older Adult Program serving over 180 individuals including those with developmental\/physical challenges.<\/p>\n

Women’s Care Center<\/strong> – $35,000
\nHelp women choose life for their babies and access quality health care & health education.<\/p>\n

Blessed Catherine Kasper Award for Outstanding Service to the Poor
\nAssociated Churches of Fort Wayne and Allen County<\/strong> – $1,500
\nFor “A Baby’s Closet” to purchase safety car seats and baby cribs, so lowincome families can adequately and safely care for their infants.<\/p>\n

Big Brothers Big Sisters of Northeast Indiana<\/strong> – $1,500
\nTo support structured Sports Buddies activities designed to encourage active and healthy lifestyles among poor and powerless youth on the mentor waitlist.<\/p>\n

Community Harvest Food Bank<\/strong> – $1,500
\nTo support Community Harvest Food Bank’s hunger relief initiatives in Allen County to help the organization meet the needs of the hungry.<\/p>\n

Early Childhood Alliance<\/strong> – $1,500
\nTo provide baby food, including infant formula, cereal, and juice, for Early Childhood Alliance’s Downtown Children’s Center, which primarily serves low-income working poor families.<\/p>\n

Fort Wayne Urban League<\/strong> – $1,500
\nTo purchase science curriculum and equipment for Fort Wayne Urban League’s academic programming in the After School Academy.<\/p>\n

The Guardian Angel Society<\/strong> – $1,500
\nFor the tuition assistance program to help innercity students in Fort Wayne enroll and stay in the City’s Catholic Elementary Schools.<\/p>\n

The Reclamation Project<\/strong> – $1,500
\nTo support The Reclamation Project’s outreach to assist the refugee community with transportation issues through the provision of bus passes, as well as underwriting driver’s training through Safeway.<\/p>\n

Society of St. Vincent DePaul<\/strong> – $1,500
\nFor providing low-income persons with round trips to medical\/therapy appointments in the Care Van.<\/p>\n

Three Rivers Right to Life Educational Trust Fund<\/strong> – $1,500
\nTo fund the Diaper Giveaway Project for lowincome families.<\/p>\n

Dr. Louis and Mrs. Anne Schneider Fellowships for Continuing Health Care Education
\nHealthVisions of Fort Wayne<\/strong> – $1,850
\nTo underwrite LaDonna Redmond to speak on food injustices and provide a $750 sponsorship for the 5th Annual Meeting of the Allen County Health Disparity Coalition.<\/p>\n

Turnstone Center for Children and Adults with Disabilities<\/strong> – $2,000
\nTo provide a training program and tools for Turnstone’s adult day services staff.<\/p><\/blockquote>\n
