News release from Super Shot:
Super Shot Reminds Families of Kindergartners to Get Shots early and Avoid the Summertime Rush
(April 14, 2013) – It?s Kindergarten Roundup time, and Super Shot wants to remind families of children entering Kindergarten to get their children?s shots now. Don?t wait until busy back-to-school time.
Over 900 Kindergarten Roundup Reminder/Recall postcards have been sent asking Super Shot families to get their children immunized. Personal calls are also being placed to these families as a reminder. The CDC states a reminder/recall program is the most effective initiative a program can undertake to keep children up-to-date.
The Indiana State Department of Health Immunization Division states the following requirements for school entry:
- 3 doses of Hepatitis B
- 5 doses of DTaP (Diptheria, Tetanus and Pertussis)
- 4 doses of Polio (Inactivated Polio)
- 2 doses of MMR (Measles, Mumps and Rubella)
- 2 doses of Varicella (Chickenpox) (or physician documentation of disease, including month and year)
- 2 doses of Hepatitis A
Families can access their child?s shot records by going to https://eportal.isdh.in.gov/MyVaxIndiana/Default2.aspx and using a PIN number from their provider.
